RE: Permission to join school excursion for Jane D. Smith
I, Dolores S. Smith, mother of Jane D. Smith, grade 4, am writing to permit my child to travel with the school on a day’s outing to the National Zoo on June 15, 2010.
I understand that there will be 5 Glory Hill High School teachers accompanying 30 Grade 4 students on that trip which will take 2 hours. I am assured that all safety precautions will be taken to ensure the full security and safety of my child and other on this trip. With that, I shall release all indemnity against Glory Hill High School if any mishap should befall my child during the trip.
I shall expect my child to reach the school for my pick up on the same day at 5 pm after dropping her at 7 o’clock in the morning.
