Subject: Maternity Leave Request Letter

Respected Madam,

I am writing you this letter to request for sanction of my maternity leave. I wish to start the leave from next week as you know that my due date is in August 2016. I have attached all the medical details and other documents that you will require for verification of the same.

I wouldn’t have asked for such an early leave had it not been for my health conditions. Due to work, I have been taking undue stress which the doctor suggests isn’t a good thing for me. So, I have been advised for proper rest by the doctor only. You shall find his letter attached to letter.

I shall be on leave from 5th June until the time it seems fine for me and my baby to join the work. I know I am in an important position in the company and so I shall complete all the necessary task before leaving. Also, I shall be handing over my share of work to one of my colleagues from accounts department. Her name is Aarti Kushwaha and she is doing very good at her current position. Once you sanction my leave, I shall brief her about the duties she has to perform, and I am sure you won’t be disappointed by her.

Thanking you in advance,

Yours Sincerely,

Charu Dubey,
