Formal Leave Letter

Introduction:

A letter for leave is written when an employee seeks a break from work usually due to a personal reason. The usual process for requesting a formal leave from work is to discuss one’s situation with their manager or human resources department. Whatever the reason is for taking the leave, it’s important to put the request in a right way. Not only will a rightly worded letter increase the odds of getting the leave request approved, but will also go a long way towards making sure that the working relationship with the boss or the employer stays active during the time away.

In the letter, one should provide the reason for the need of leave which is usually a personal issue. One should also mention the things they are willing to do to help prepare for the absence and offering to remain available for questions during the absence. The dates of departure and return to work that one is anticipating should also be mentioned explicitly in the letter.

Subject: Request for Leave.

Dear Sir,

I, Christopher W. Pascal, am writing this letter to inform you that I need to take sick leave from work due to severe influenza and currently unable to talk or breathe properly, due to a swollen throat. The doctor has suggested that I remain off work until the end of this month, ultimately taking a leave for the whole week. I have attached a letter from the doctor, who is treating me, to help you confirm the time off I need to properly recover without risking my health or that of my co-workers. I sincerely apologize for any inconvenience that my absence from work might cause.

However, I should be able to occasionally check my emails and do some work, including the few projects which I had almost completed, from home when possible until I come back to the office. When I return on the first day of next month, I wish to do my best to catch up to any work I may have missed during my absence.
Please let me know if you would need further verification from my doctor.

If I may require additional time off, I shall let you know at the soonest. Please feel free to contact me at your convenience should you have any questions work related or require further verification of my sick leave request. Thanking you for your prompt attention to this matter.
