Employee Maternity Leave Letter


Employee Maternity Leave letter is a letter written by a female staff who is pregnant and in need of maternity leave while her delivery date is approaching. Hope this letter reaches to you at the best of your health.

I am writing this letter to inform you that I am due for a medical checkup and my delivery as per the doctor’s report. Having checked on the doctor’s availability and my work responsibility, I would like to request a leave from the month of July to the month of December to go for my medical checkup and delivery which is dated on DD/MM/YYYY. I am extremely sorry for not informing this to you before one month. Hope you could understand my situation and do the needful.

I shall most likely be back to work from next year. In case otherwise, I shall call in to inform the office of the situation. I shall ensure that all my work responsibilities will not be affected by this checkup request.

I am attaching a copy of the doctor’s report along with this form, and if you need a medical certificate for the same, I can bring it also if you let me know about it before two days of my delivery date which is aforementioned in the letter. Sorry for the inconveniences caused due to my leave.
