Day Off Request Letter

Introduction

A Day off request letter is written by an employee who wishes to take a day off from work. The individual must report to the immediate superior or to the management to make this one day off request, stating the reason giving notice where possible, else making apologies for the short notice due to unforeseen circumstances. It is considered courteous to inform the concerned person about the leave well in advance.

In a corporate set up leaves are typically of 3 kinds- a) Sick Leave b) Paid Leave c) Casual Leave. Each employee is designated certain number of off days in a year. These leaves are then categorized into the above mentioned. The employee is entitled to specific number of days under each category. Once the leaves have been exhausted for every extra leave the employee takes she or he will have their salary cut for the day the leave has taken.

Dear Sir,

Re: Day off request

I am writing to request your kind consideration in approving my intended one day off tomorrow, January 31, 2017. I have written to the Human Resource Department, however they asked me to seek permission from the Department Manager first. Only once you give your heads up for the leave will the Human Resource Department be able to approve my leave.

The reason I request for this one day off is that I have to take my mother for her doctor’s appointment at the General Hospital. She was supposed to be accompanied by my dad who has fallen sick.

Since it is a government hospital, the queue may be long and the service slow. Hence, I am unsure how long I would take in accompanying my mom on her annual check up. Due to how time-consuming it is I feel I will not be able to attend office tomorrow.

I do apologize for the short notice given, but I was only informed today of the circumstances. Kindly consider this as my casual leave. So far I have taken only one leave in the year that is on the 2nd of January 2017.
