Dear Sir,

Re: Day off request

I am writing to request your kind consideration in approving my intended one day off tomorrow, January 31, 2011.

The reason I request for this one day off is that I have to take my mother for her doctor’s appointment at the General Hospital. She was supposed to be accompanied by my dad who has fallen sick.

Since it is a government hospital, the queue may be long and the service slow. Hence, I am unsure how long I would take in accompanying my mom on her annual check up.

I do apologize for the short notice given but I was only informed today of the circumstances.

Thank you for your kind understanding on my request. I look forward to a positive response.

Yours faithfully,

Sarah Mills
